JOB DESCRIPTION:
Tigerlily Foundation’s Patient Navigator will ensure young women are provided full-lifecycle support from breast cancer diagnosis through treatment and beyond.  This individual will also track and oversee Tigerlily Foundation’s Program Managers to ensure satisfaction of program offerings to the young women, we support.  This position requires strong organizational, coordination, communications skills, and compassion. This position will receive a stipend.

RESPONSIBILITIES:

  • This individual will be responsible for overseeing Tigerlily Foundation (Tigerlily) program offerings and program managers.
  • Track participants from initial program entry, through Tigerlily programs, including our Peer Education Through Awareness and Leadership (PETALs), Buddy, Buddy Bag, Meals at Home, Fearless Females and Stage 4 Programs.
  • Work with Tigerlily leadership to ensure programmatic objectives are met.
  • Conduct monthly, quarterly and annual assessments of programs, identify weaknesses, create mitigation plans and implement to improve quality of programs
  • Keep updated documentation of program history[y and materials
  • Build and maintain relationships within community, faith-based and health organizations that we aim to support, to include physicians, nurses, social services, radiation, hematology/oncology staff, and other patient navigators by calling, visiting, etc.
  • Asses and analyze data, and manage program objectives, interventions and outcomes to ensure they are met, and assess impact on target population
  • Provide referrals to patients, if needed and connect patients to community, health and social support resources.
  • Identify ways to better target Tigerlily-specific patient population
